Job summary

Beacon Hospice, Amedisys/Optum, seeks an RN Case Manager to coordinate compassionate hospice care at home across Damariscotta and coastal areas. Assess patients, partner with the interdisciplinary care team, and develop plans that meet each patient’s needs.

Current RN license in the state(s) you are assigned to work.
1+ year of nursing experience.
Current CPR certification.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops and maintains individualized patient care progress records with accuracy and timeliness.
Records pain/symptom management changes/outcomes as appropriate.
Educates patients, families, and caregivers on appropriate patient care techniques.
Regularly communicates with the clinical manager and care team.
Performs on-call responsibilities and on-call services to patients/families as assigned.
