Transitions Hospice is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team in moments that matter.

This role emphasizes compassionate, in-home end-of-life care with 24/7 support, conducting comprehensive assessments, developing personalized care plans with physicians, and ensuring accurate documentation while educating families and participating in on-call rotations.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Coordinate patient care and guide the interdisciplinary team.
• Build and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Serve as the communication bridge between families, physicians and community partners.
• Educate patients and caregivers to promote confidence and support.
• Participate in on-call rotation per team policy.
