Job summary

BAYADA Home Health Care in Pennsylvania is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients at home across Montgomery and Bucks counties. You will work within a multidisciplinary team to provide palliative care that allows patients to remain in their homes with comfort and dignity.

The role offers flexible schedules (full-time, part-time, per-diem) with daytime hours and on-call as needed, plus 1:1 training and ongoing support from a larger care team

Qualifications

• Active and current RN license in PA.
• Hospice experience preferred.
• Willingness to travel within designated territories.

Responsibilities

• Make home visits to hospice patients in Montgomery County and Bucks County, PA.
• Administer medications, wound care, treatments, and comfort care per plan.
• Monitor patient conditions and report changes to Clinical/Client Services Manager.
• Follow doctors' orders and document accurately.
• Occasional on-call after hours and/or weekends.
