Job summary

Puget Sound Hospice is seeking an experienced Hospice RN Case Manager to join our team serving Thurston County, WA. You’ll be the liaison between patients, families and the clinical team, delivering skilled nursing, developing care plans, and guiding families through the care journey.

This full-time role offers a salary range of $85,000–$110,000 per year DOE, a supportive work environment, benefits, and opportunities for growth. Active WA RN license, BLS, and hospice experience preferred.

Qualifications

• Current Registered Nurse (RN) license in Washington state.
• Home health or hospice experience preferred.
• Current BLS certification (AHA).
• Licensed driver with insured transportation.
• Compassion, communication skills, and clinical expertise.

Responsibilities

• Deliver skilled, compassionate care to patients in their homes.
• Perform assessments and develop individualized care plans.
• Educate and support patients and families/caregivers.
• Collaborate with physicians, LPNs, CNAs, and community providers.
• Participate in on-call rotations for urgent or standby visits.
