You will develop and implement individualized care plans, monitor patient well-being, and collaborate with a multidisciplinary team to ensure compassionate end-of-life care.

The role requires a Georgia RN license and CPR certification, with a focus on delivering high-quality care, maintaining confidentiality, and supporting families through the hospice

Qualifications

• RN licensure in Georgia required.
• 1 year hospice experience preferred.
• CPR certification required.

Responsibilities

• Assumes full nursing responsibility for the Plan of Care for each hospice patient.
• Continuously assesses performance and quality of care; makes adjustments to improve outcomes.
• Maintains patient confidentiality and communicates effectively with patients, families, and staff.
