Good Samaritan Hospice is seeking compassionate RN Case Managers for Hospice and Home Health within Roanoke, VA. Join a mission-driven team delivering patient-centered care focused on dignity and quality of life at end of life.

Responsibilities include assessments, care coordination, and updates to interdisciplinary plans of care, with on-call rotation and ongoing education to support patients and families.

Qualifications

• Graduate of an accredited nursing program.
• Virginia RN license (or compact).Current BLS/CPR certification.
• Strong clinical assessment, communication, and decision-making skills.
• Comfort with EMR and mobile documentation tools.

Responsibilities

• Provide case management for assigned hospice patients.
• Conduct comprehensive assessments and ongoing visits.
• Develop, implement, and update individualized Plans of Care with the IDT.
• Manage pain and symptom control in collaboration with the physician.
• Educate patients and families on medications, symptom management, and end-of-life care.
• Coordinate care with physicians, IDT members, and caregivers.
• Provide skilled nursing interventions including wound care and IV therapy.
• Participate in IDT meetings, on-call rotation, and quality improvement initiatives.
