Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Registered Nurse Case Manager for Hospice in Dare and Currituck counties. The role is full-time with an office in Nags Head, NC, providing patient-centered hospice care and coordination from 8am to 5pm, Monday through Friday.

Ideal candidates have an accredited nursing degree, RN licensure, hospice experience, and a strong commitment to clinical excellence and documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a reliable vehicle.
• Possess and maintain CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
