Job summary

CorsoCare Hospice is seeking a Registered Nurse Case Manager to provide compassionate nursing care to terminally ill patients in home settings. You will support families, coordinate with the Hospice team, and ensure high-quality patient care in Canton/Livonia areas.

The role requires an accredited nursing degree, current Michigan RN license, and at least 2 years of RN experience, with strong independent judgment and crisis response abilities. A flexible schedule and on-call duties are included.

Qualifications

• Graduate from an accredited school of nursing.
• Current RN license to practice in the state.
• Minimum 2 years of RN experience, preferred.
• Active patient contact within past 3 years, preferred.
• Ability to work independently and make accurate judgments.
• Ability to supervise others appropriately.
• Ability to respond to crises outside of a hospital setting.
• Acceptance of and adaptability to different social, racial, cultural and religious modes.

Responsibilities

• Regularly assesses and reassesses the nursing needs of Hospice patients.
• Provides dietary counseling and Hospice nursing services.
• Initiates nursing procedures for Hospice care and safety.
• Observes signs and reports changes to physician/IDG.
• Teaches, supervises and counsels patients and families.
• Supervises and trains nursing staff.
• Develops and updates the care plan with IDG.
• Performs nursing procedures per doctor’s orders.
• Attends team conferences and maintains records.
• Follows Hospice policies and HIPAA requirements.
• Looks after on-call coverage and scheduling.
• Coordinates plans for SNF/NF/ICF/MR patients.
• Organizes work schedule and time management for meetings.
• Complies with infection control policies.
• Assists with orientation, training, and other duties.
