Qualifications

• RN licensed to practice in Michigan.
• Graduate from an accredited nursing program.

Responsibilities

• Regularly assesses and reassesses the nursing needs of Hospice patients.
• Provides Hospice nursing services, treatments and preventive procedures.
• Teaches and counsels patient and family on care.
• Coordinates plan of care with IDG and community resources.
• Attends team conferences and maintains patient records.
