Job summary

Rochester Regional Health Home Care is seeking a Registered Nurse Case Manager to oversee comprehensive nursing care for an assigned patient group. You will assess needs, develop and implement a care plan, coordinate interdisciplinary services, and supervise PDNs as needed to ensure quality home-based care.

Responsibilities include coordinating discharge plans, documenting care per agency standards, and communicating patient status changes to the team.

Qualifications

• RN licensure and nursing degree required per NYS regulations.
• BLS certification required.

Responsibilities

• Identify and prioritize health problems based on assessment.
• Develop and implement an interdisciplinary Plan of Care with input from patient/caregivers, in collaboration with the physician and care team.
• Coordinate patient care including complex cases to ensure delivery of services and supports.
• Complete assessments and documentation per agency standards (e.g., OASIS/HIS as applicable).
• Plan and prioritize care for an assigned caseload; ensure services meet the plan of care.
• Communicate changes in patient status to the care team and trigger appropriate actions.

Education

Diploma or Associate’s Degree in Nursing

RN license in New York State
