Job summary

Transcend Hospice in the United States seeks a Registered Nurse to provide skilled nursing care in patients’ homes, coordinating with the interdisciplinary team, patient, family, and referring agencies to ensure comprehensive care delivery.

The role emphasizes assessing needs, developing and updating the patient plan of care, educating families, and ensuring timely interventions while traveling locally as required. Strong communication and interpersonal skills are essential.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current state licensed as a Registered Nurse, current State Driver’s License
• Two years’ experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of the patient’s physical, functional, psychosocial, and environmental needs.
• Implement the individualized POC and revise plan as necessary.
• Consulting with and educating patient/family regarding disease process, care needs, and end-of-life care.
• Initiating preventive and rehabilitative nursing procedures.
• Preparing progress notes showing progress toward goals.
• Coordinating all patient/family services with the IDT.
• Referring to other services as needed and using them effectively.
• Informing physician of changes in patient needs and outcomes of care.
