Ascend Hospice in Reading, MA is now hiring an RN Case Manager. Starting salary is $93,600.

We offer flexible and personalized scheduling, a great work-life balance, and a Monday–Friday schedule with NO OVERNIGHTS OR WEEKENDS. You will complete initial assessments of patients and families to determine hospice care needs, regularly re-evaluate nursing needs, develop care plans, collaborate with the healthcare team and families, counsel on end-of-life needs, initiate appropriate nursing

Qualifications

• State Certified RN license required.
• Associate's Degree in Nursing.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patients nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide a care that is continuous and well-coordinated.
• Counsel the patient and family in meeting end of life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition.
• Supervises the Hospice Aides.
