Job summary

Hospice of North Alabama is seeking a compassionate Registered Nurse Case Manager to join our Scottsboro team. You will provide patient-centered hospice care, develop care plans, administer medications, and educate families while coordinating with an interdisciplinary team.

Qualifications

• Graduate of an accredited nursing school with current RN licensure.
• One year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver’s license and insured vehicle.
• CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
