Elizabeth Hospice seeks an RN Case Manager to provide professional nursing care for adults, pediatrics, and perinatal patients in the North San Diego County area. You will coordinate care with the interdisciplinary team, supervise LVNs/HA, and ensure timely charting and communication with IDT and primary physicians.

The role includes shift coverage Monday–Friday 8:00–5:00 with two weekend shifts monthly and on-call requirements, along with travel to patient visits.

Qualifications

• Associates degree in nursing with 1 year of recent nursing experience or a BSN from ACEN-accredited program.
• CA RN license required.
• Current CA driver’s license with proof of auto insurance.

Responsabilités

• Develops written plan of care in coordination with an interdisciplinary Team (IDT).
• Oversees plan implementation and evaluates adjustments to meet patient/family needs.
• Supervises LVNs and Homehealth Aides per policy and regulatory requirements.
• Maintains timely patient documentation using point-of-care electronic charting.
