Job summary

Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager to join our compassionate nursing team serving Westchester and surrounding areas. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey.

As part of our interdisciplinary team, you will collaborate with the Clinical Director, provide education, support, and compassionate advocacy, and participate in on-call coverage.

Qualifications

Active RN license in good standing.
At least 1 year RN experience in med-surg/acute care; hospice exp a plus.
Computer proficient; EMR experience.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Provide direct nursing care focusing on comfort and quality of life.
Deliver nursing care using evidence-based practices.
Educate patients and families to participate in care plans.
Participate in on-call duties as part of team.
