Good Samaritan seeks a full-time RN Case Manager, Hospice, in Nisswa, MN. The role requires leading the interdisciplinary team, assessing new admissions, and delivering compassionate end-of-life care.

Qualifications

• Bachelor's degree in Nursing strongly preferred.
• Currently holds an unencumbered RN license in the state where practice occurs.
• Two years’ clinical experience preferred.
• Must possess a valid driver’s license.

Responsibilities

• Provide care to assigned patients in a hospice setting.
• Lead and coordinate the interdisciplinary plan of care.
• Assess residents at admission and ongoing per policy.
• Document nursing care clearly and communicate with patients, families, and team.
