Silverado Hospice is seeking an RN Case Manager in the Temecula area to coordinate patient care and develop individualized plans for hospice and palliative care across the community.

You will lead with clinical expertise and compassion, guiding patients and families through the end-of-life journey with dignity. This role is full-time and based in California, with competitive benefits and growth opportunities.

Qualifications

• Registered Nurse with a valid state license.
• Minimum of 1 year RN experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Must clear criminal background check, physical, and drug screening.
• Valid driver’s license and reliable transportation; willing to drive to patient locations and work outside usual hours.

Responsibilities

• Develop detailed Plans of Care for assigned patients and coordinate services.
• Collaborate with the Interdisciplinary Group, patient, family, and physician.
• Provide skilled hospice and palliative care.
• Conduct comprehensive nursing evaluations and assessments.
• Provide field supervision to LVNs and Home Health Aides.
