Job summary

Legacy Hospice in Poplar Bluff, MO seeks a Registered Nurse Case Manager (RNCM) to deliver compassionate end-of-life care at patients' homes. You will coordinate treatment with the interdisciplinary team and develop individualized care plans.

Requirements include an active RN license in Missouri, CPR/BLS, and at least 1 year in hospice or home health. BSN/CHPN preferred; on-site role with competitive salary and benefits.

Qualifications

• RN license in the state of employment is required.
• BSN or CHPN preferred.
• Minimum 1 year of hospice/home health nursing experience.
• Active CPR/BLS certification and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and develop care plans.
• Manage a caseload of hospice patients and coordinate care with the IDT.
• Administer medications per physician orders and monitor treatments.
• Educate patients and families on symptom management and end-of-life care.
• Maintain timely clinical documentation and participate in IDT meetings.
• Provide emotional support to patients and caregivers and ensure continuity of care.
