Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide holistic in-home care, coordinating a personalized plan with patients and families. You will lead end-of-life conversations, manage symptoms with empathy, and work with a care team to ensure smooth transitions.

You will perform assessments, develop plans, administer medications, and educate families while on-call. A nursing degree and active RN licensure are required, with hospice experience preferred.

Qualifications

• RN must have an accredited nursing degree (ADN/BSN/MSN) and current licensure.
• Minimum 1 year of acute care experience; hospice/home health experience preferred.
• Experience administering medications and developing individualized care plans.
• Ability to educate patients and caregivers on care techniques, medications, and symptom management.
• Willingness to rotate on-call duties and maintain reliable transportation with a valid license.

Responsibilities

• Perform initial and ongoing comprehensive assessments to determine hospice needs.
• Develop and update individualized care plans with the patient and family as needs evolve.
• Administer medications and treatments as prescribed by the physician.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Collaborate with the care team to ensure seamless care transitions and on-call coverage.
