Job summary

Ohio Living is seeking a skilled RN Field Staff - Hospice to plan, coordinate and deliver compassionate hospice care in patients' homes and healthcare facilities across Ohio.

The role requires an Ohio RN license and at least 1 year of acute care or medical-surgical nursing; experience with Medicare-certified home health is preferred. Proficiency with EMR, Windows, and Microsoft Office is expected. On-call availability 24/7 is required.

Qualifications

• Current Ohio RN license required.
• At least 1 year of acute care or medical-surgical nursing experience.
• Medicare-certified home health experience preferred.
• Proficiency with Windows, Microsoft Office, and internet.
• EMR experience preferred.

Responsibilities

• Provide ongoing assessment and nursing care to patients in homes and health care facilities.
• Develop and implement a comprehensive care plan addressing current needs.
• Provide effective pain and symptom management for patient comfort.
• Communicate with other health care providers about changes in condition and needs.
• Assess patient and caregiver needs and provide necessary education.
• Prepare for IDG by reviewing care plan and reconciling medications.
