Hospice Plus is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate, high‑quality end‑of‑life care in patients' homes and alternative settings. You will assess, plan, implement, and coordinate care with an interdisciplinary team to support patients and families throughout the hospice journey.

Qualifications

• Must hold an active RN license in Texas.
• Hospice or home health experience preferred.
• Strong patient education and communication skills; able to document clearly.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and visit frequency based on patient needs.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease process, self‑care, and end‑of‑life care.
• Document timely clinical records and ensure compliance with regulations.
• Support continuity of care and transitions between care levels as needed.
• Participate in on‑call and patient support activities as applicable.
