Job summary

Bristol Hospice in Oregon is seeking a dedicated RN Case Manager to provide compassionate, whole-person hospice care, advocating for patients and families while coordinating interdisciplinary teams across Klamath County. You will develop individualized care plans, monitor vitals, administer physician-ordered treatments, educate families, and document progress throughout the care journey.

This full-time role is Monday–Friday, 8am–5pm, with coverage in Klamath County.

Qualifications

• Active Registered Nurse license in the state of practice.
• CPR certification is current.
• Valid driver's license and reliable transportation.
• Strong observational and written/verbal communication skills.
• Willingness to travel within the coverage area and collaborate with the care team.

Responsibilities

• Develop and implement individualized care plans based on patient history and current needs.
• Deliver one-on-one support to patients and families, ensuring comfort and dignity.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor vital signs and document changes accurately.
• Administer physician-ordered medications and treatments with empathy.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
