• RN license in Texas, in good standing.
• Hospice experience preferred.
• Strong clinical assessment and critical‑thinking skills.
• Excellent communication with patients, families, and care teams.
• Ability to work independently while remaining connected to a team.
• Bilingual English/Spanish a plus, reflecting the community we serve.
• Compassionate, dependable, and patient‑focused approach to nursing.

• Complete comprehensive patient assessments and develop individualized plans of care.
• Serve as the primary clinical liaison for patients, families, physicians, and hospice team members.
• Provide skilled nursing visits, symptom management, and compassionate end‑of‑life care.
• Educate patients and families on disease progression, comfort measures, and available support services.
• Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members.
• Monitor patient status and adjust care plans as needed.
