Qualifications

• Current RN licensure in Georgia.
• Experience in hospice care required.
• Ability to travel across the service area with reliable transportation.
• Strong teamwork and communication skills.
• Ability to learn and use an Electronic Medical Records (EMR) system.

Responsibilities

• Cares for a group of patients in homes, SNFs and assisted living facilities.
• Participates as part of an interdisciplinary team with psychosocial assessments.
• Manages visits, documentation and scheduling while coordinating with care providers.
• Travel across the service area to patient locations.
