Job summary

Suncrest/Brighton Hospice in the Atlanta area is seeking a Registered Nurse Case Manager to deliver comprehensive care to patients in home and facility settings. You will collaborate with an interdisciplinary team and coordinate services to support patient goals.

The role requires Georgia RN licensure, strong communication, travel across the service area, and experience in hospice. This full-time position offers competitive pay ($85k–$95k) plus a monthly bonus, plus benefits and PTO.

Qualifications

Current Registered Nurse (RN) licensure (in good standing) in the state of Georgia.
Collaborate daily with an interdisciplinary team.
Work 1-1 with patients while supported by the care team.
Work independently with phone, email and meetings, and follow hospice rules.
Strong time management for visits, documentation and meetings.
Ability to learn an electronic medical records system.
Willingness to travel across the service area with reliable transportation.
Previous hospice experience required.

Responsibilities

Provide care and services for a group of patients and visit patients where they reside.
Visit patients in private homes, Skilled Nursing Facilities, and assisted living facilities.
Be part of an interdisciplinary team providing psychosocial assessments and interventions.
Utilize knowledge of community resources to support patient care.
