Job summary

Optimal Care, Carmel, IN is seeking a Hospice RN Case Manager to deliver compassionate, skilled nursing care to patients and families at the end of life. You will coordinate with an interdisciplinary team to provide comprehensive, dignified care in patients' homes and facilities.

The role offers autonomy in caseload management, travel within the service area, and collaboration with physicians, social workers, and chaplains.

Qualifications

• Graduate of an accredited Diploma/ADN/BSN nursing program.
• Current State License as a Registered Nurse.
• Minimum 1 year of nursing experience in a clinical setting.
• Reliable transportation with automobile insurance.
• Willingness to travel extensively within the service area daily.
• BSN preferred.
• Experience in hospice or home health is preferred.

Responsibilities

• Provide skilled hospice nursing care and symptom management.
• Coordinate care with interdisciplinary team and physicians.
• Assist with hospital discharges to home or facility settings.
• Document visits within 24 hours and maintain care plans.
• Support patients and families with education and bereavement when needed.
