Job summary

Optimal Care invites a dedicated Hospice RN Case Manager to join our clinician-owned team in Carmel, IN. You will deliver skilled nursing care, coordinate comprehensive cases, and collaborate with an interdisciplinary team to honor patient goals and family needs.

This role requires autonomous caseload management, extensive travel within the service area, and commitment to compassionate, high-quality end-of-life care.

Qualifications

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum 1 year of nursing experience in a clinical care setting.
• Reliable transportation with valid automobile insurance.
• Willingness to travel extensively within service area daily.

Responsibilities

• Provide skilled hospice nursing care including initial evaluations and ongoing assessments.
• Coordinate comprehensive care plans and transitions from hospital to home or facility settings.
• Collaborate with physicians and the interdisciplinary team in IDG meetings and care conferences.
• Maintain thorough documentation within 24 hours of visits and ensure coordination across settings.
• Support patients and families with education on symptom management and end-of-life care.
