Job summary

Jobtailor seeks a registered nurse to manage hospice nursing cases, primarily admissions, and to develop, implement, monitor, and update individualized care plans. You will coordinate with physicians, extenders, clients, caregivers, and interdisciplinary teams.

Responsibilities include initial assessments, medication administration, direct client care under the State Nurse Practice Act, patient education on end-of-life needs, re-evaluating plans, coordinating discharge planning, and supervising

Qualifications

• RN qualification and ability to provide direct client care as defined in the State Nurse Practice Act.
• Ability to complete client, caregiver, and client representative assessments.
• Ability to develop, implement, update, and revise individualized plans of care.
• Ability to administer medications and treatments as prescribed by the physician/physician extender.
• Ability to communicate with physicians/physician extenders and obtain orders as required.
• Ability to educate and mentor clients, caregivers, and client representatives.
• Ability to coordinate care with interdisciplinary teams, community health providers, and facility staff.
• Ability to supervise LPN/LVN and HH/Hospice aides.
• Ability to attend interdisciplinary team meetings and coordinate discharge planning.
• Availability Monday-Friday, 8:30a-5p, with occasional on-call weekends and typically two holidays per year

Responsibilities

• Manage hospice nursing cases, primarily admissions
• Develop, implement, monitor, and update individualized plans of care
• Communicate and coordinate with physicians, physician extenders, clients, caregivers, client representatives, community health providers, facility staff, and interdisciplinary teams
• Complete initial and comprehensive physical assessments and obtain illness histories
• Initiate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed
• Provide direct client care under the State Nurse Practice Act
• Educate and support clients and caregivers on care needs and end-of-life issues
• Re-evaluate nursing needs and revise care plans as client status changes
• Coordinate discharge planning
• Attend interdisciplinary team meetings and ensure care and treatment are communicated and documented
• Instruct, evaluate, develop plans for, and supervise LPN/LVN and HH/Hospice aides
