Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1 holistic care in patients' homes. You will partner with an interdisciplinary team to develop individualized plans, guide end‑of‑life discussions, and ensure patient comfort with empathy and professionalism.

The role requires an RN with acute care experience, ability to educate patients and families, and readiness to rotate on-call duties while maintaining high-quality home-based hospice care.

Qualifications

• Current RN license (state of practice)
• 1+ years of acute care experience
• Assess hospice needs and develop care plans
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care and meds
• Rotate on-call duties and ensure smooth transitions
• Reliable transportation, valid driver's license, insurance
