Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in their homes, coordinating with an integrated care team to craft individualized care plans.

The role requires RN with 1+ years of acute care experience; hospice/home health experience preferred, and on-call duties are part of the schedule. This full-time position offers 8:00am–5:00pm hours, with comprehensive benefits including health, dental and vision insurance,

Qualifications

• 1+ years of acute care experience required; hospice/home health experience preferred.
• RN must be licensed and in good standing.
• Strong communication and teamwork skills are essential.

Responsibilities

• Develop individualized care plans with patients and families.
• Coordinate care with the integrated care team.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and ensure smooth care transitions.
