Job summary

St Croix Hospice in Cedar Falls, IA is seeking a Registered Nurse Case Manager to provide 1:1 holistic home-based care. You will partner with an interdisciplinary team to assess patients, develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.

The role requires RN licensure (ADN/Bachelor's/Master's) and 1+ year acute care experience; hospice/home health experience is preferred.

Qualifications

• RN should hold an associates, bachelor’s, or master’s degree in nursing (RN).
• 1+ year of acute care experience required; hospice/home health experience preferred.
• Ability to perform comprehensive assessments and develop individualized care plans.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.

Responsibilities

• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
