Spanish Cove At Home is seeking an RN Hospice Case Manager to plan and oversee hospice care for patients in the Home Hospice program. You will assess needs, manage symptoms, and provide support to families in a compassionate, interdisciplinary setting.
The role requires a current Oklahoma RN license, BLS/CPR certification, and 1+ year in med-surg/geriatric or long-term care nursing, with willingness to participate in 24/7 coverage.

Education and/or Experience:
This employee must be a graduate of accredited school of Nursing, Current RN nursing license in good standing with the State of Oklahoma, BLS/CPR certification, and a minimum of 1 year of experience in med-surg, geriatric or long-term care nursing.Prefer a minimum of 5 years’ supervisory experience.
Computer Skills:
AOD software experience preferred along with basic computer skills.
