Spanish Cove At Home in Oklahoma is seeking a compassionate RN Hospice Case Manager to lead personalized hospice care plans for residents.
You will perform skilled nursing assessments, coordinate with an interdisciplinary team, educate families, and ensure documentation complies with policies. This role may require on-call coverage and weekend availability to support patients and families during challenging times.

This employee must be a graduate of accredited school of Nursing, Current RN nursing license in good standing with the State of Oklahoma, BLS/CPR certification, and a minimum of 1 year of experience in med-surg, geriatric or long-term care nursing.Prefer a minimum of 5 years’ supervisory experience.
AOD software experience preferred along with basic computer skills.
