Job summary

The Elizabeth Hospice is seeking an RN Case Manager to provide professional nursing care across pediatric, adult, and perinatal patients. Working with the IDT and families, you will assess, plan, implement and evaluate care, ensuring effective communications and care coordination across internal and external customers.

Responsibilities include developing care plans, supervising LVNs/HCAs, maintaining documentation, and participating in 24/7 coverage as needed.

Qualifications

  • Spanish, Farsi or Tagalog speaking preferred.
  • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.

  • Directs care provided by LVN and HA in accordance with patient’s plan of care.
  • Supervises services provided by LVN and HA per TEH policy and regulatory requirements.
  • Communicates all changes to IDT and primary MD.
  • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice.
  • Maintains timely patient documentation using point of care electronic charting.
  • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
  • Provides health care instruction to patient and family members.
  • Orients, instructs, and trains assigned personnel.
  • Participates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation).
  • Performs admission assessment and admission when necessary.
  • Adhering to the E-way standards is a requirement in performing all job duties.

Description

The Elizabeth Hospice RN Case Manager is primarily responsible for providing professional nursing care to assigned patients, which includes adults, pediatric and perinatal. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice.

Hourly Pay: $45.00-$61.00 per hour. Hourly pay based on years of RN licensure.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m.

On Call: 2 shifts monthly (may require weekend call)

Work Location: North San Diego County and South Riverside County- Travel Required for patient visits.

Qualifications:

1. Must have Associate's degree plus one (1) year of experience as a professional nurse within the past three (3) years or baccalaureate degree in nursing from a program accredited by the Accreditation Commission for Education in Nursing (ACEN) or current accreditation program) required.

2. Professional nursing experience preferred in hospice, oncology, emergency room, pain clinic or medical-surgical nursing.

3. Current CA RN license.

4. Current CA driver’s license with proof of auto insurance.

5. Current CPR certification and negative TB skin test.

6. Prefer certification as hospice nurse.

7. Pediatric experience preferred.

8. Spanish, Farsi or Tagalog speaking preferred.

9. Computer skills (word processing), use of adding machine, calculator, fax, photocopier.

10. Because this is a field position, must be able to drive in various conditions, including but not limited to winding/steep roads, night driving, and rural areas, and be able to enter homes with pets.
