Job summary

Care Initiatives Hospice seeks a compassionate Registered Nurse (RN) Case Manager to provide comprehensive patient-centered care in homes and community settings across Storm Lake, Pocahontas, Sac City and surrounding areas.

You will assess needs, develop care plans, and coordinate interdisciplinary teams to honor patient wishes, with a strong emphasis on dignity and comfort for patients and families. Competitive pay and robust benefits offered, plus opportunities for growth.

Qualifications

• Active Iowa RN license is required.
• Active CPR certification is required.
• Hospice or palliative care experience is preferred.
• Two years of verifiable nursing experience (RN/LPN) preferred.
• Valid driver’s license and ability to travel between multiple locations within the region.
• Home internet access is required, with CHPN certification possible within 24 months.

Responsibilities

• Deliver high-quality patient care in the patient’s residence using nursing knowledge, assessments, and procedures.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies and procedures.
• Conduct continuous visits and interdisciplinary assessments of patients.
• Provide accurate clinical documentation and care coordination across teams.
• Counsel patients and families, educating on plan of care and available services.
• continually update the care plan to meet patient-specific needs.

Care Initiatives is looking for a RN to help care for patients in Storm Lake, Pocahontas, Sac City & surrounding areas.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
