Job summary

Comfort Hospice in Pahrump, Nevada is seeking a dedicated Hospice RN Case Manager for a full-time role that travels throughout the Pahrump area. You will assess patients and families, develop care plans, provide skilled nursing care, educate families, and maintain thorough documentation.

The ideal candidate holds an active Nevada RN license with at least 2 years of nursing experience, hospice or home health preferred, and a valid driver's license with reliable vehicle.

Qualifications

• Current RN license in Nevada is required.
• Minimum 2 years nursing experience; hospice/home health preferred.

• Ability to work independently and deliver patient-centered care.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares commitment to excellence.
• Participate in holiday on-call rotation as needed.
