Job summary

Montgomery Hospice, Inc. is seeking a skilled RN to provide primary nursing care and case management for hospice patients in the community. You will develop and implement nursing plans of care, coordinate with other clinicians, and document thoroughly in the EMR system.

We require Maryland RN licensure, with BSN preferred, and at least two years of relevant clinical experience, ideally in critical care or hospice. This role includes a retention bonus and a competitive salary.

• Location: Ft. Washington, Accokeek, Suitland, Clinton, Upper Marlboro, Brandywine, etc.
• Wage Range: $85,000/Yr-$105,000/Yr *$10,000 RETENTION BONUS OFFERED*

Position Summary:

Functions as a member of an interdisciplinary team, responsible for providing case management and primary nursing care to assigned patients and formulating and implementing nursing plan of care consistent with standards of practice.

Minimum Qualifications:

• Graduation from an accredited nursing program. B.S.N preferred.
• Current State of Maryland Nursing License.
• Hospice certification preferred.
• Two years of recent clinical experience, preferably with critical care or previous experience in a hospice or homecare program.
• Requires strong critical thinking skills and EMR experience.
• Possess excellent verbal and written communication skills, and ability to communicate effectively with patients and families under stress.

Duties and Responsibilities:

• Provides primary nursing services for assigned patients in the community.
• Provides case management for hospice patients.
• Assesses, plans, implements and evaluates nursing plans of care for assigned patients.
• Completes all documentation using electronic medical records system.
• Maintains contact with patient’s primary physicians.
