Position Overview: Unified Care Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager to join our dynamic team. As a Registered Nurse Case Manager, you will play a pivotal role in providing holistic and patient-centered care to our hospice patients and their families. Your expertise, clinical skills, and compassionate approach will help ensure the highest quality of life for patients during their end-of-life journey. Responsibilities
  • Conduct comprehensive assessments of patients" physical, emotional, and psychosocial needs, collaborating with the interdisciplinary team to develop personalized care plans.
  • Coordinate and manage patient care, ensuring that interventions are consistent with patients" wishes, goals, and the hospice philosophy.
  • Provide skilled nursing care, pain and symptom management, and emotional support to patients and their families in accordance with established protocols.
  • Collaborate closely with physicians, social workers, chaplains, and other team members to address the diverse needs of patients and families.
  • Communicate effectively with patients, families, and the care team, providing education about the hospice process and supporting informed decision-making.
  • Monitor patients" condition, update care plans as needed, and ensure timely documentation in electronic medical records.
  • Participate in regular interdisciplinary team meetings, contributing insights and expertise to enhance patient care delivery.
  • Serve as a resource for healthcare providers, patients, and families, offering guidance on pain management, symptom control, and end-of-life care.
  • Engage in continuous education and training to maintain and enhance your clinical skills and knowledge.
  • Actively participate in team events, fostering a collaborative and supportive work environment.
