Job summary

Cadre Family Companies, through Inspire Hospice and Palliative Care, seeks a compassionate Hospice Registered Nurse to deliver in-home and facility-based care within our service area.

As part of the interdisciplinary team, you will assess patients, manage symptoms, educate families, and coordinate with physicians to ensure dignified end-of-life care. This role emphasizes integrity, empathy, and collaboration, with opportunities for professional growth and a supportive culture.

Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of one year of nursing experience.
• Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.

Responsibilities

• Provide bedside hospice care to patients and families.
• Deliver compassionate care in patients' homes, assisted living communities, and skilled nursing facilities.
• Manage patient symptoms with clinical expertise and empathy.
• Educate patients and families about disease progression and end-of-life care.
• Collaborate with the interdisciplinary team to ensure coordinated hospice services.
• Document assessments and care plans in the EMR.

About the Role

As a Hospice Registered Nurse , you'll be an essential part of our care team, providing compassionate, high-quality care to patients and families in the comfort of their homes, assisted living communities, and skilled nursing facilities. You'll use your clinical expertise, compassion, and understanding of the hospice philosophy to help patients receive the comfort and support they deserve.

What You'll Do

• Provide high-quality bedside care to patients and their families.
• Deliver compassionate care in patients' homes, assisted living communities, and skilled nursing facilities.
• Manage patient symptoms with clinical expertise and empathy.
• Support patients and families throughout their hospice journey.

Educate & Support Patients and Families

• Provide education on disease progression, end-of-life care, self-care techniques, and nutrition.
• Help patients and families understand their care and what to expect throughout the hospice journey.
• Provide compassionate support while honoring each patient's needs and wishes.

Collaborate with Your Care Team

• Work closely with the interdisciplinary team to ensure comprehensive care.
• Communicate and collaborate with other members of the care team to support patient needs.
• Help ensure patients receive coordinated, compassionate hospice care.

Document & Coordinate Care

• Accurately document assessments, progress notes, care plans, and physician orders.
• Maintain complete and accurate patient records.
• Communicate patient needs and changes in condition appropriately with the care team.

Qualifications

Required Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of practice.
• Minimum of one (1) year of nursing experience.
• Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service area.

Preferred Qualifications

• Previous hospice, palliative care, home health, oncology, or case management experience.
• Strong clinical assessment and care planning skills.
• Experience educating patients and caregivers.
• Proficiency with electronic health record (EHR/EMR) documentation.
• Excellent communication, collaboration, and relationship-building skills.
• A compassionate, patient-first mindset and a commitment to delivering dignified, person-centered end-of-life care
