Job summary

Cadre Family Companies, including Inspire Hospice and Palliative Care, is seeking a Hospice Hospital Liaison to connect hospitals with our hospice services and build strong clinical relationships.

You’ll collaborate with physicians, discharge planners, social workers, and senior care facilities to expand referrals and deliver compassionate, patient-centered care.

Qualifications

• Healthcare experience required.
• Experience in hospice, home health, post-acute care, or related fields.
• Excellent communication and interpersonal skills.
• Strong problem-solving and follow-through.
• Familiarity with local senior resources and advance directives.
• Proficient with basic computer skills.

Responsibilities

• Develop and strengthen relationships with referral sources (hospitals, physicians, discharge planners, social workers).
• Connect with leaders of skilled nursing facilities and assisted living facilities to expand referrals.
• Build community partnerships to raise awareness of Inspire Hospice services.
• Represent Inspire as a trusted community advocate and provider.

About the Role

As our Hospice Hospital Liaison , you'll be the trusted connection between Inspire Hospice and the hospital community. You'll build strong relationships with hospital teams, physicians, discharge planners, social workers, and other healthcare professionals while helping raise awareness of the exceptional hospice services we provide.

What You’ll Do

• Develop and strengthen relationships with potential referral sources.
• Connect with physicians, hospital discharge planners, social workers, and leaders of skilled nursing and assisted living facilities.
• Build strong community partnerships that increase awareness of Inspire Hospice's services.
• Serve as a trusted representative of Inspire throughout the local community.

Develop Business Opportunities

• Scout and develop new business territories.
• Identify new business opportunities and strengthen existing relationships.
• Support sales and marketing strategies designed to expand awareness of Inspire's hospice services.
• Help connect patients and families with the hospice programs and services they need.

Represent Inspire in the Community

• Promote awareness of Inspire Hospice and Palliative Care and the services we provide.
• Develop relationships with community resources that support seniors and individuals facing chronic or terminal illness.
• Help educate referral sources and community partners about hospice services and available resources.
• Represent Inspire with professionalism, compassion, and a strong commitment to patient care.

Qualifications

• Previous healthcare experience is required.
• Experience in hospice, home health, post-acute care, or related fields.
• Excellent communication and interpersonal skills.
• Strong problem-solving skills with impressive follow-through.
• Familiarity with local senior support resources, advanced directives, and challenges faced by chronically or terminally ill patients and families.
• Proficient in fundamental computer skills and knowledge of basic business software.

Preferred Qualifications

• College degree in business, marketing, or nursing is a plus.
