Job summary

RNInk is seeking a full-time RN Case Manager to coordinate and oversee hospice patient care in the Dallas area south of Downtown Dallas, including Cedar Hill, DeSoto, Red Oak, and Duncanville. The RN will develop and implement individualized plans of care and provide clinical assessment and symptom management as part of the interdisciplinary team.

• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.
