Job summary

H.T. Prof Executive Search in the Dallas area is seeking a full-time RN Case Manager for a hospice provider.

The nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families. Ideal candidates hold a Missouri RN license with at least two years of RN experience in a clinical setting; hospice experience is preferred.

Qualifications

• Must hold a current Registered Nurse license valid for practice in Missouri.
• Valid driver's license.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of hospice experience preferred.
• Reliable transportation and automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.
