Job summary

Suncoast Hospice, a part of Empath Health, seeks an RN Case Manager in the North Hillsborough area to provide compassionate end-of-life care on a Monday–Friday 8a–5p schedule. You will lead symptom management, assess needs, coordinate care with an interdisciplinary group, and educate patients and families about palliative options.

Minimum one year of acute-care nursing experience; Florida RN license and CPR required; Hospice RN experience and CHPN encouraged within three years; BSN/MSN preferred

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid CPR certification required.
• Minimum one year experience in acute-care Hospital environment providing direct patient care; Hospice RN preferred.
• CHPN certification strongly encouraged within three years of employment.
• BSN or MSN preferred.

Responsibilities

• Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• Develops, implements and updates the individualized plan of care with IDG; initiates referrals and coordinates services as needed.
• Educates patient/resident, family and caregivers regarding disease process, palliative interventions, care giving, dying process and safety practices.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated RN Case Manager to work with us in the North Hillsborough area while providing exceptional care on the following schedule: Monday-Friday 8a-5p.

What You’ll Do

• Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG); develops implements and updates the individualized plan of care, initiates appropriate preventive and rehabilitative procedures and initiates referrals to other services; requests complimentary services, as needed.
• Educates patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices.

What You’ll Need
