Job summary

Suncoast Hospice, part of Empath Health, is seeking an RN Case Manager to support patients in the North Hillsborough area. The role follows a Monday–Friday 8:00 a.m. to 5:00 p.m.

schedule and emphasizes compassionate end-of-life care within a dedicated interdisciplinary team. Qualifications include Florida RN licensure, CPR certification, and at least one year of acute-care hospital experience; CHPN certification is encouraged within three years.

• Graduate of approved nursing program.
• Current and unrestricted Florida RN license.
• CPR certification required.
• Minimum one year acute-care hospital experience; hospice RN preferred.
• CHPN certification encouraged within three years of employment.
• BSN or MSN preferred.

Responsibilities

• Under physician direction, manages discomfort and relieves symptoms for palliative/end-of-life care.
• Performs initial and ongoing assessment of patient needs across physical, functional, psychosocial, and environment factors.
• Collaborates with Interdisciplinary Group to develop and update the care plan; initiates referrals and supplementary services as needed.
• Educates patient, family, and caregivers about disease progression, palliative care, and safety practices.

Suncoast Hospice, a part of Empath Health is seeking a caring and motivated RN Case Manager to work with us in the North Hillsborough area while providing exceptional care on the following schedule: Monday-Friday 8a-5p.

What you\'ll Do:

• Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG); develops implements and updates the individualized plan of care, initiates appropriate preventive and rehabilitative procedures and initiates referrals to other services; requests complimentary services, as needed.
• Educates patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices.

What You\'ll Need:
