Job summary

COH Holdings seeks a Hospice RN Case Manager for MN/WI-Western. This full-time role coordinates hospice care with the care team and families, ensures plans of care are current, and conducts ongoing assessments.

You will collaborate with physicians, the IDG, and contracted services, maintaining timely clinical notes and on-call coverage. Eligibility includes RN licensure in the state, with preferred hospice/home care experience.

Qualifications

• Must be a licensed RN in the applicable state.
• Hospice, home health, or long-term care experience preferred.
• Strong communication and documentation skills required.

Responsibilities

• Coordinates hospice care with team members, patients and families to meet needs.
• Evaluate ongoing effectiveness of hospice services for patient/family.
• Consult with attending physician and Hospice Medical Director on changes in plan of care.
• Coordinate patient services with IDG and contracted services as needed.
• Submit complete, accurate clinical notes and weekly visit schedules.
• Participate in IDG and case conferences; keep plans of care current.
• Handle on-call duties (including holidays) per policy.
• Conduct initial and ongoing comprehensive patient assessments.
• Observe responses to treatments and report changes to physicians.
• Supervise Hospice Aide visits at least every 14 days; chart nursing notes.

POSITION TITLE: Hospice RN Case Manager- MN/WI-Western

Full time

Pay Wage: $85,000+ DOQ

Nursing Management:

• Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
• Consult with the attending physician and Hospice Medical Director concerning changes in patients’ condition requiring order changes and/or changes in the plan of care.
• Coordinate patient services with other members of the IDG as well as contracted services, when needed.
• Submit complete, accurate and relevant clinical notes in a timely manner.
• Submit patient visit schedule weekly.
• Participate in IDG and case conferences.
• Take responsibility for ensuring patients’ plans of care are kept current.
• Performs on-call duties (including holidays) per policy and agency need.
• Perform death visit per agency’s policy.
• Conduct an initial as well as ongoing comprehensive assessments of the patients.
• Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, as well as changes in the patient's physical or emotional condition.
• Supervise and evaluate the care given by the Hospice Aide at a minimum of once every 14 days and as needed.
• Chart those services rendered to the patient by the staff nurse and changes that have been noted in the patient's condition and/or family and home situation, makes revisions in the nursing care plan as needed, records supervisory visits conducted with the Hospice Aide, evaluates patient care and progress and closes charts of discharged patients.
• Graduate of an accredited school of nursing
• Must be registered and licensed with the State of working as a RN, and must maintain current unencumbered license with the Board of Nursing

Qualifications:

• Experience within a long term care facility or homecare or hospice preferred
• Excellent judgment and decision-making abilities and problem solving skills
• Must be a licensed driver with an insured automobile in good working order.
• The ability to deal tactfully and use good communication and customer service skills with staff, residents and their families both verbally and in writing
• Basic computer skills and ability to use electronic records
• Must pass criminal background from Department of Human Services at the time of hire and every 4 years after
• Must be screened for and pass TB test
• Must pass a drug screening test.
