Ohio's Hospice in Troy, Ohio is seeking a dedicated RN Hospice Care Manager for day shifts. The role covers patients in homes and facilities within Miami County and surrounding areas, with weekend rotation every sixth weekend and one holiday per year.

The candidate will plan and deliver nursing care, work within a multidisciplinary team, and maintain professional standards while supporting end-of-life care for patients and families.

Qualifikationen

• Graduate of an accredited nursing school; ASN/BSN preferred.
• Active RN license in Ohio without board actions.
• Two years acute care nursing experience preferred.
• CHPN certification preferred; required within two years of employment.
• CPR Certified and eligible to drive as required.
• Computer skills to document services and care.
