Job summary

Ohio’s Hospice is seeking an experienced Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families in Newark, OH, including Mount Vernon and Centerburg. This field-based role serves patients at home and in facilities, managing a 25–30 patient caseload and coordinating care with the interdisciplinary team.

The RN Care Manager will assess patient needs, manage symptoms, educate families, and advocate for comfort and quality of life in alignment with hospice

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in the State of Ohio, in good standing.
• Current CPR certification.
• Valid driver’s license with a safe driving record.
• Reliable transportation and ability to travel throughout the assigned service area.
• Ability to meet state-required automobile insurance requirements.
• Basic computer skills and ability to accurately document patient care.
• Successful completion of required pre-employment screening, including background check and drug screening.

Responsibilities

• Manage an assigned caseload of approximately 25–30 hospice patients.
• Provide skilled nursing care to patients in their homes and in healthcare facilities.
• Complete comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient conditions and recognize changes that may require adjustments to the plan of care.
• Assess and manage pain and other symptoms to promote comfort and quality of life.
• Coordinate care and communicate effectively with physicians and members of the interdisciplinary team.
• Educate patients, families, and caregivers regarding disease progression, medications, symptom management, and available hospice services.
• Provide emotional support and serve as an advocate for patients and families.
• Communicate changes in patient condition and care needs to the appropriate members of the care team.
• Maintain accurate and timely documentation of assessments, interventions, medications, communications, and patient outcomes.
• Coordinate services and resources to support patients across home and facility settings.
• Follow applicable nursing standards, hospice regulations, organizational policies, and the established plan of care.
• Participate in weekend and holiday rotations as scheduled.
• Travel throughout the assigned service area to provide patient care.
