Job summary

Ohio’s Hospice is seeking a Hospice RN Care Manager for the Dayton, OH area. This full-time day shift role involves direct patient care to individuals in facilities and homes within Dayton/Trotwood. You will lead nursing care using the nursing process and collaborate with families and the care team.

Required Ohio RN license with two years acute care experience; ASN and BSN preferred. CPR certified; CHPN preferred and must be obtained within two years.

Qualifications

• CPR certification required.
• Two years acute care nursing experience preferred.
• Active RN license in the State of Ohio without board actions.
• CHPN certification preferred; required within two years of employment.

Responsibilities

• Plan and deliver care to patients using the nursing process (assessment, planning, interventions, evaluation).
• Interact effectively with patients, families and interdisciplinary team members.
• Deliver care to patients in facilities and homes within the Dayton/Trotwood area.
• Maintain standards of professional nursing practice.
• Able to drive day, night, or adverse weather as needed.

Hospice RN – Making every moment matter

This is Full Time day shift M-F 8am-5pm position. This is a driving position delivering care to patients living in facilities and homes within the Dayton/Trotwood area

As a Hospice RN you will plan and deliver care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interact with patients, families and other interdisciplinary team members while maintaining standards of professional nursing. We provide superior care and superior services to patients at their end-of-life journey. Only those who have a heart for hospice will succeed.

Hospice RN Qualifications:

RN's - Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years of acute care nursing experience preferred.

• CPR Certified
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
• Computer skills are sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
