Qualifications

• Requires Associate’s degree in nursing from an accredited school.
• Bachelor’s degree in nursing preferred; state licensure as RN.
• Experience in general medical-surgical nursing and hospice/home care desired.

Responsibilities

• Coordinate care across settings with interdisciplinary team.
• Perform first-level clinical reviews per criteria and Utilization Plan.
• Ensure timely admission, discharge, and resource utilization.
• Document in EMR accurately and timely.
• Provide education to patients and families about care plans and end-of-life care.
• Participate in admission processes and 24/7 on-call duties.
