Halifax Health is seeking a Registered Nurse - Hospice (Patient Care Coordinator) to lead the interdisciplinary team across patient care settings. You will provide clinical direction, coordinate care, and ensure compliance with regulations, under the supervision of the Director of Nursing.

Qualifications

• Florida RN license currently in good standing.
• Associate degree in nursing is required.
• Bachelor's degree is preferred.
• Minimum 3 years in home health or medical-surgical nursing.
• Supervisory experience is required.
• Strong interpersonal communication with patients, families and staff.
• Demonstrates professional development, accountability and leadership.
• Valid driver's license with good driving record.

Responsabilités

• Provide clinical direction to interdisciplinary team with Director of Nursing support.
• Interview, orient, and assign staff to meet patient needs.
• Provide ongoing staff education and clinical direction.
• Supervise and counsel team members as appropriate.
• Participate in management decisions and organizational goals.
• Manage and review budget for errors and cost containment for the assigned team.
• Participate in Quality/Performance Improvement and Utilization Review.
